- Enrollment Forms
- Official Release of Records
- Home Language Survey
- Current High School Transcript (10th -12th grade students)
- 8th Grade Promotion Certificate (8th grade students)
- Immunization Records or exemption form
- Student Device Agreement
- Birth Certificate or acceptable forms.
- Copy of Individualized Education Plan or 504 Plan (if applicable)
